How it works:

Initial Evaluation

Your initial evaluation includes a comprehensive health history and exam.  We discuss your weight history, past medical, and surgical history, lifestyle and identify medical conditions that may be contributing to your weight along with a review of your current medications.

Design a Plan for You

You work 1-1 with Dr. Collman  who also has additional training with the American Board of Obesity Medicine.  Once your history has been reviewed, we then design a plan with you based on your health history and lab work. However, in most cases, we combine lifestyle changes with medications to maximize success. There are many medications designed to help with weight loss and we pride ourselves in knowing which one is best for you.

Accountability

Over the years we have found that the most successful patients are the ones that are seen regularly. In many cases it is simple accountability that keeps patients motivated. Your package includes bi-weekly check-ins and communication with your doctor to help you reach your goals.

Weight Loss Membership FAQ:

  • Weight Loss Membership Program is 6 month minimum commitment. Monthly payment options are available.
  • The fee is $199 per month. This fee does NOT include the cost of medication.
  • There is a one time nonrefundable enrollment fee $75 per individual.

  • We do not accept insurance; all services are self-pay. The treatment fee is considered an out of pocket expense. However, if you have commercial insurance, medications may be billed to your insurance. About 60-70% of patients who are eligible for our program get insurance coverage on their medication.
  • Patients who are approved through their insurance plans for their medication will be responsible for their medication copay, which will be an out-of-pocket expense. We encourage patients to be sure to ask your insurance plan if they cover weight loss medications.
  • Some insurance plans exclude weight loss medications from their coverage plan, and therefore, even if a patient meets the requirements for these medications, their insurance companies may not cover them.
  • We will attempt to process a prior authorization through your commercial prescription drug plan.
  • If the prior authorization is approved you will need to call in to your insurance to determine what out of pocket costs will be for the medication ( if any)  and schedule a consultation to get your prescription sent. 
  • You will also need to check with your local pharmacy to confirm its in stock prior to your appointment. 
  • If your insurance company does not approve the prior authorization for the medication  and you wish to pay out of pocket,  we will discuss the best options to help you save on medications.
  • If your insurance company does not approve the prior authorization and you do not wish to pay out of pocket, you can receive a treatment refund from Prōse Medical minus the $75 enrollment fee. 
  • Yes, we accept FSA and HSA as a form of payment. Simply use your FSA or HSA card as your form of payment on file. 
  • Yes, Financing is available through CareCredit
  • No, Prōse Medical is a direct care practice that works with individuals to optimize their health. This is one of the memberships our clinic offers to help patients develop a plan to get healthier.
  • It depends on each patient and your weight loss needs.  After discussing your goals and an evaluation, Dr. Collman will create a special treatment plan for you to reach your optimal weight. She will offer prescriptions for FDA approved medications for weight loss (including GLP-1 Medications like Wegovy, Mounjaro)  and compounded Semaglutide/Tirzepatide when appropriate.  
  • Every patient has a unique metabolic health profile and weight loss goal. This is not a one-size-fits-all program, and the number of pounds varies from one person to the next.  However, our patients lose an average of 10 to 15% of their starting body weight and successfully maintain that weight loss. Typical results vary and on average significant weight loss is seen after three-six months.
  • We traditionally will spend at least an hour with new patients to learn more about them. Once we have established a plan, we’d like to see each client monthly ( either virtually or in person)  to measure their weight, body composition ( if in person) , blood pressure, and review how they’ve responded to the treatment plan.  We provide bi-weekly check in’s via text directly with your physician. 
  • No, the weight loss program is considered a specialty service. Specialty services are focused visits and do not include primary care membership services such as wellness exams, sick visits, or acute care services. You do not need to be a primary care member to be apart of the weight loss membership.  
  • If you change insurance companies while on the treatment and your new insurance carrier does not approve these medications, you will not receive a refund once you start the program. We suggest that you check with your prospective insurance carrier to make sure that you can continue taking the medication on your new plan.
  • If you wish to pay out of pocket, you will also be eligible for the accessibility-program ( see above). 
  • Currently, Dr. Collman works with patients in Fl, GA, and VA.

Yes, if you are not near the greater Orlando area for in person visits, you can do this program virtually.
