What is Prōse Medical?

Prōse Medical is a membership-based practice on a mission to providing consistent high-quality primary care and wellness services. The members are top priority and membership plans offer personalized and accessible care. By offering options such as direct access to your doctor, same and next day appointments with longer visits, members have time to have their concerns addressed in a relaxed setting.

How is Prōse Medical different?

Your visits will never feel rushed and new members are scheduled for an entire hour to allow us to get to know them and their healthcare goals. You will also know exactly how much your healthcare will cost with no surprises. As the practice is limited to just a few hundred members, and not thousands, you will be able to contact Dr. Collman when you need advice and be able to schedule appointments quickly when needed.

Is there a sign up fee to join?

Yes, for the primary care membership, there is a one time non-refundable enrollment fee of $150. This fee contributes to initial on-boarding process, obtaining medical records, and a comprehensive initial physical examination. The enrollment fee is waived with annual enrollment.

How much does the monthly membership cost?

Our Primary Care Membership fee is $150 per month for the first adult member.

Can children become members of Prōse Medical?

At this time, Dr. Collman is seeing patients aged 13 years and older. Kids can be easily added to an accompanying adult membership for a discounted monthly fee of $75 per child and receive the same benefits. We are not able to provide routine vaccinations but will happily work with you to arrange these. We do require that at least one paying adult membership accompany a child membership.

Does Prōse Medical accept commercial Insurance?

We see patients who have insurance. We use your insurance for labs, medications, and imaging services. We don’t charge co-pays or deductibles but instead, use a membership model to provide higher-quality care and bill a flat monthly membership fee.

As a member of Prōse Medical, do I still need insurance?

Yes as we do not replace your existing health insurance. There are certain types of care we cannot provide for you, such as surgeries and hospital stays, and you would still need insurance to cover these emergency situations. We suggest that members continue or opt to carry both a major medical plan and a health savings account (HSA) to ensure financial help should hospitalization or referral to a specialist be necessary.

Can I use my HSA or FSA to pay for membership ?

If you have an HSA or Flex Spending Account (FSA) , you may be able to use these funds to help pay for your annual membership. Consult your tax advisor for guidance on your particular circumstances.

What if I need labs or imaging?

Arrangements will be made for you to get your labs at a facility of your choice. Some routine labs are also included in your membership fee. If you have insurance, lab tests such as pap smears and mammograms are still covered by your choice of insurance and can be billed to them. If you do not have insurance, the cost of other additional labs will be given prior to collection. Charges for labs will be added and applied to your preferred method of payment for membership. Dr. Collman has negotiated on your behalf with labs and imaging centers for the best self-pay prices and will confirm prior to scheduling. Charges for imaging will be required upfront with the imaging center.

What happens if I need to go to the hospital or see a specialist?

In instances where those types of care are required, we will work with your hospitalist during your hospital stay as well as work closely with any specialist when such care is needed. In some cases, depending on the level of care and the expertise required, we may rely on those specialized caregivers to manage your treatment. We do encourage you to carry insurance for these kinds of needs, as we are not considered an insurance plan.

What if I need medical attention when I am away from home?

Many illnesses can be diagnosed and treated with a simple conversation by phone, email, text, or telemedicine (virtual visit). As a result of the nature of our practice, you are able to speak with Dr. Collman wherever you are or easily schedule a virtual visit. If you have a condition that requires in-person evaluation and treatment, Dr. Collman will direct you to appropriate nearby care, and coordinate follow-up when you return home.

Will I benefit from Prōse Medical if I don’t require frequent medical attention?

Yes! Unfortunately, most medical care has become very reactionary, something we use as a reaction to having symptoms or being sick. While we always provide sick care for our patients, it is also important to be proactive and create plans for staying healthy. Certainly, everyone benefits from healthcare tailored to them. The traditional healthcare model encourages you NOT to seek care until you are very sick, but with Direct Care, you can seek care at any time, preferably sooner rather than later, and we can actually prevent things from happening instead of waiting for them to happen.

What if I decide to cancel my membership?

We understand that your needs may change. If you need to cancel, we require a 30 day notice prior to your next bill. To allow your doctor time to get to know you and address your healthcare needs, we require a six-month minimum of membership. If you signed up for an annual term and decide to cancel, your remaining balance will be refunded.

How do I become a member?

At this time, we are open for enrollment which you may access on our membership page or by clicking the “Enroll to Become a Member” tab above.

Please remember that space is limited so we can devote the highest standard of personalized care and service to each member. We accept members on a first-come, first-served basis. Once the enrollment limit is reached, we will establish a waiting list, so we encourage you to act quickly.
